Unconditional Home Care Agency is a personal care or private duty agency that provides non-medical services to help individuals who need assistance with their activities of daily living (ADLs).
Activities of Daily Living include but are not limited to:
- Medication Reminders
- Transferring from a toilet or bed
- Light Housekeeping
- Transfer from a chair
- Assistance with Bathing
- Preparing Meals
- Transportation
- Getting Dressed
What Is a Home Care Worker?
Home Care Workers are referred to as Direct Care Workers (DCW). DCW’s provide assistance with the Activities of Daily Living (ADL) in a patient’s residence or home. Direct Care Workers assist patients with tasks such as meal preparation, medication reminders, laundry, light housekeeping, and companionship. No medical degree/education or license is required. Their hourly pay ranges from $9 to $13.
What Can We Do to Start Home Care Services?
- Start an Enrollment Application Process.
- Call Unconditional Home Care Agency at 267-231-9307.
- An assigned worker will answer all your questions and schedule a Home Visit Appointment.
- During the face-to-face appointment, Enrollment State forms will be completed.
- Enrollment paperwork will be submitted to the State Office on your behalf.
- An Assessor will call you and visit you in your home to complete a level of care determination.
- The Independent Enrollment Broker meets with you in your home to complete a needs assessment form.
- The County Assistance Office will complete the financial eligibility determination.
- The Patient Agreement form will be signed with the provider Unconditional Home Care Agency.
